This form is for professionals who want to progress I Matter Training and who plan to self fund or whose organisation, school, GP practice, PCN or local authority has agreed funding in advance to support access to I Matter training.
This form is for self-funding confirmation or for for full or partial funding agreements.
Once you submit the form:

we confirm your agreed way forward

we confirm eligibility with the funding partner if appropriate

we enrol you onto the appropriate programme

you receive joining details by email
You do not need to arrange payment yourself if your place is fully funded. If your place is self-funded or partially funded, the outstanding amount will be payable before you access the course materials.
Funded places are subject to confirmation of eligibility and partner agreement.
